This page houses resources and materials provided to communities across Texas exploring and planning to install the Help Me Grow (HMG) Model. These technical assistance efforts were conducted by the HMG National Center between September 2020 and August 2021 in support of Model implementation at the state and local level.
July 2021 Session Materials
A Collaborative & Strength Based Approach for Implementation of Successful HMG Care Coordination
Effective, powerful support to families with young children, delivered by well-trained HMG Care Coordinators, is a fundamental part of the HMG Model. Without well-informed, positive, persistent, well-supported, and empathetic Care Coordinators, families cannot be linked successfully to the supports and services they need.
Care Coordination as included in the HMG Model is set apart from more traditional information & referral practices by key activities such as follow-up with families to ensure linkage was successful and that all needs are met, and in proactively looping feedback to the child and family’s medical providers. Additionally, essential attributes of HMG Care Coordination include motivational interviewing, active listening, responding reflectively, taking a parent/caregiver-centered approach, operating from a strengths-based method, cultural responsiveness, critical and creative reasoning, providing trauma informed care, as well as employing an empathetic and non-judgmental communication style.

May 2021 Session Materials
Framing HMG’s value to current and potential system partners is essential for growth and sustainability. To move partners from abstract buy-in to tangible participation, we utilized this technical assistance session to support Texas communities in their efforts to individualize the value proposition for your highest priority audiences. In this session, you will examine your affiliate’s unique partner landscape and pinpoint HMG leverage points to build traction.
This presentation focuses on messaging the value proposition of HMG to advance efforts such as building stakeholder buy-in, engaging partners, seeking funding, and marketing.

January 2021 Session Materials
This session focused on ways to set up a new affiliate’s HMG Core Component Work Groups and Leadership in order to maximize efficiency and ensure continuous communication. Communities became more familiar with the HMG Readiness Assessment tools and their potential value to guiding Work Group efforts to develop recommendations and ultimately make decisions about local implementation strategy. The session also included basics on the RAPID decision-making theory as a support in coming to final strategic plans for HMG implementation.

November 2020 Session Materials
This session was devoted to the HMG National Center’s Roadmap to Replication, a toolkit designed to clearly articulate what the necessary steps and activities a new HMG affiliate must take in order to build HMG with full implementation and fidelity to the Model.
