Advancing Goal Concordant Care through Help Me Grow Implementation Meet the Project Partners: Healthy Outcomes from Positive Experiences (HOPE)

To provide the highest-quality care to young children and their families, all early childhood providers must acknowledge parents and caregivers as partners in decision-making, and all services provided must be in concordance with the goals, needs, aspirations, and values of the family. An early childhood system that shares power with the families it serves, and whose providers implement care plans that are co-developed with families, is operating with goal concordant care at the center.

The Help Me Grow (HMG) National Center is announcing a new project, supported by The JPB Foundation, that will identify, test, and refine ways in which HMG systems can systematically elicit family goals and priorities and enable parents to co-determine goals of care with their providers.

The HMG National Center hosted a series of webinars in January 2022 to introduce the project partners, their core principles, and how they support organizations and systems in integrating those principles. In this session, project partner Healthy Outcomes from Positive Experiences (HOPE) shared some key tools and resources any HMG System might consider in their efforts to advance goal concordant care.
